Felt and Fiber Bazaar Sign Up: # of tables ____ x $50.0 per table = $______ ( payable with registration). The Bazaar will run on two days: Aug 25, 1:00 to 5:00 p.m. and Aug 29, 9:00 a.m. to 12:00 noon. 

Photo Shoot Sign Up: $75.00 per hour x # hours ____ = $ _____ (payable to the photographer at time of shoot (minimum 1/2 hour; specify total hours in quarter hour increments). There is also a fee of $7.50 per slide and $10.00 per digital image payable to the photographer.

Self Portraits in Felt sign-up: I will submit a work in the "Self Portraits" Exhibition at Felters' Fling. Approximate size: _______ (please specify if your piece needs to be hung or displayed on a table ___________________________).

Lectures, Discussion Groups, and Demonstrations: We plan to make time available on August 29th for participants to share their knowledge and special talents with others or participate in discussion groups on issues related to feltmaking. If you or your group would like to reserve time during the conference to show slides of your work, lead a discussion, demonstrate equipment or a technique please list information below (or attach separately).
