In order to provide a form that you can print out on a single piece of paper we have put on a page all by itself, with no links or navigation buttons to get you to our "Home" page, etc. Therefore, when you have printed the form, use your browser's "back" button to return here where you can click on the buttons to get to the rest of the web site.

Before printing the form, make sure your browser is set with margins of about 1/2" all around. You can do this by using the File/Page Setup menu command or something similar, depending on your browser. Then use the "File/Print" command in your browser to print the form. 

Once it is printed, fill it in and then mail it or fax it to us and keep a copy for your records. Before completing the form, please review the Fling details, starting with the Overview. Pay special attention to the class descriptions and lodging details.
